Disability living allowance mobility component at the higher rate (an award notice letter from the Pension, Disability & carers service)
Mobility component of PIP with a score of 12 or more (a decision letter from the DWP under the mobility component section)
High-rate attendance allowance (letter from the DWP and a supporting letter from a medical practitioner.
Vehicle registration number (V5 Document)
Proof of permanent residency (e.g. council tax bill/ utility bill/ letter from DWP
